Cityblock Health delivers primary care, behavioural health and social services to people on Medicaid and to dual-eligible Medicare-Medicaid members, working in dense urban neighbourhoods through a mix of clinics, home visits and community health partners. Founded in 2017 and headquartered in Brooklyn, New York, it contracts with health plans and state programmes rather than billing patients, and says it serves close to 200,000 members at roughly $2.2 billion of annualised revenue - up 77% year on year and more than four times its level at its 2021 fundraise - across 18 customers. Its internal Care Orchestration and Resourcing Engine (CORE) ranks which intervention will close which care gap for which member; the company puts CORE's predictions right 62% to 87% of the time and says adjacent AI tooling freed 44,599 clinician hours in 2026. In August 2026 Cityblock raised a $116 million Series E led by General Catalyst and simultaneously agreed to acquire Homeward, a rural-first Medicare Advantage provider with about 50,000 members, in an all-stock deal. Chief executive and co-founder Toyin Ajayi leads the company.
