Unite Us builds software and data infrastructure for coordinating what it calls whole-person care across healthcare providers, government agencies and community-based organisations, so that a referral for a social need such as food, housing or transport can be tracked to an outcome rather than lost. Its platform covers a closed-loop referral system, a resource directory, predictive analytics, care coordination, grant tracking and billing, revenue cycle management and a Self Sufficiency Score. Customers include state and local governments, hospitals and health systems, Medicaid and managed-care plans, public health agencies and nonprofits, among them North Carolina DHHS, Optum, Goodwill, Salesforce and Epic. The company reports a 96 per cent closed-loop referral completion rate and more than $250 million in community reimbursements.
